Policies

Vaccinations:Our providers strongly believe in vaccinating all children that are medically able to receive the recommended vaccines. We do not accept families that are unwilling to vaccinate as per the AAP(American Association of Pediatrics) recommendations. We do not accept religious exemptions. We do not allow the spacing out of vaccinations without a documented medical reason. If you refuse to vaccinate your child according to the recommended schedule, we ask that you find another provider who shares your views. 

No Show/Late Show to appointments:If you do not show up or arrive more than 15 minutes late to your scheduled appointment, it is considered a No Show and we may need to reschedule your appointment to another day and time. If you cancel your appointment with less than 2 hour's notice, it will also be considered a No Show. Families that accumulate 3 No Shows over a 3 year period will be at risk of being discharged from our practice. Any new patient that No shows to their first appoinment will not be allowed to reschedule and will not be accepted into the practice.

Insurance:You are responsible for notifying us of any changes to your child's insurance in a timely manner. If you do not, you may end up being reponsible to pay for any visits accrued.

Well visits: It is important that we see your child(ren) for regular head-to-toe Well Visits (physical) for us to give your child the best care. If you fail to bring your child for his/her well visit as recommended by your provider, you risk being discharged from our practice for non-compliance.

  • What's included in a Well visit?:
    • Administer all recommended immunizations
    • Reviewing your children's medical, family and social history
    • Reviewing allergies and current medications list. 
    • Measuring vital signs (blood pressure, heart rate, temperature, weight, height, Oxygen saturation, BMI)
    • Reviewing growth charts and developmental questionnaires
    • Screening for Anxiety, depression, high risk behaviors, drug abuse when age appropriate. 
    • Full body exam including genital and breast exam, scoliosis assessment.
    • Performing a spot vision test starting at 1 year old. Urine and hearing test starting at 4 years old.
    • Performing an Oral Health risk assessment and Fluoride application for children 1 to 4 years of age not seen by the dentist yet. 
    • Performing Lead and Hemoglobin by capillary testing(fingerstick) at the office. Order blood work to outside lab

  • Well visit schedule by age:

    • Newborn
    • 1 month
    • 2 months
    • 4 months
    • 6 months
    • 9 months
    • 12 months
    • 15 months
    • 18 months
    • 24 months
    • 30 months 
    • 3 years and yearly after this until 18 years old
